"I certify that I am at least 21 years of age and agree to abide by the rules of Chain of Command and any venues hosting FUCR 2009. I understand that photos may be taken during the run and that it is my responsibility to notify the person taking the photo if I do not desire to have mine made public, otherwise a photo release is hereby granted. I, my heirs, and assigns voluntarily agree to hold harmless Chain of Command, all establishments hosting or participating in FUCR 2009, Members, guests from any and all liability for loss or injury to my person and/or property. I further agree to defend and indemnify Chain of Command, all establishments hosting or participating in FUCR 2009 and their Officers, members and guests for any and all loss or injury caused by or related to any action by me whether negligent or intentional. I agree not to carry nor use any illegal drugs or substances during this event.

I further understand admission to this event is contingent upon acceptance of my application by Chain of Command, who reserves the right to accept or reject any application solely at its discretion. I accept that violation of any of the provisions of this agreement shall constitute sufficient grounds for my immediate explulsion from the premises."
